Every person has a story. Their care plan should reflect it.
Care Plan Stories is an online learning collaborative for anyone involved in supporting older adults—including direct care staff, clinicians, leaders, family members, and advocates.
Participants learn practical ways to create care plans that honor each person's strengths, preferences, relationships, routines, and goals while supporting meaningful, person-directed care.
