That’s why we’re excited to share Care Plans Worth Reading, a practical, strengths-based framework that helps teams develop care plans that reflect the person, not the task and diagnosis list.

This collaborative supports teams to:

• Honor each elder’s life rather than just document it

• Eliminate institutional language and approaches

• Create plans that support well-being and meet regulatory requirements

• Strengthen collaboration between care partners, elders and families

This learning collaborative offers a meaningful care plan story alternative to support relationships while meeting regulations.
